Q: I entered the Signature and Date but it didn't save. What should I do?

A: Please use a different browser from whichever one you are using. When you switch to a different browser, such as the latest version of Chrome, Firefox, or Safari, make sure the browser has javascript and cookies enabled and private browsing disabled. If this does not work then please use a different device. For example, if you are using your home computer use a computer at the library instead.

Q: How do I provide a signature online?

A: By typing your name in the Signature field, you are signifying that you are the person who completed the form and the information provided is factually and honestly presented. In effect, by typing your name and agreeing to submission, you are "signing" the document.
